Lakeland Care Services provides 24‑hour assisted and customized living for older adults and adults with disabilities. Our homes are in five cities across the south and east metro, with options for adults 55+ and 18+.

Care that fits each person

Our homes are small, so staff get to know each resident well. Care is planned with the resident, their family and their case manager, and changes as their needs do.

Staff are on site 24 hours a day, every day.

  • Personal care

    Help with bathing, dressing, grooming and getting around the home safely.

  • Medication assistance

    Trained staff keep medications on schedule and watch for changes in health.

  • Meals and daily routines

    Home-cooked meals and help with the routines that make a day feel normal.

  • Appointments and outings

    Help getting to medical visits, family events and activities in the community.

Each home is set up a little differently. Tell us in your referral what the person needs, and we'll match them to the right one.

How referrals work

Every step happens by email, so families and case managers always have a written record.

  1. Email the referral

    Send the person's details, waiver type, home needs, support plan, MnCHOICES assessment and any other documents that show their needs to referrals@lakelandcareservices.com.

  2. We review it

    We look at care needs and preferred city, and check them against our current openings.

  3. We reply with next steps

    If there's a good fit, we set up a visit to the home.

  4. Move-in

    We agree a move-in date with the family and case manager and handle the paperwork together.
